Well, I think you are drawing the wrong conclusion. You should use COMSPEC to point to any command shell you like. That shell should be compatible to cmd.exe, though. Are you sure it is related to Salamander's pane, rather than to what is displayed in that pane?
Any chance some Unicode might be involved, somewhere in a folder name or the file name you right click, or even in something that would be displayed in the context menu?

What happens if you switch panes in AS (Menu Left or Right, Swap (or Ctrl+U))? Is it still the left pane that's not working? What happens if you move Salamander's window so the left pane will be where the right pane was on your screen. Still not working? Any software running that hooks or otherwise...

Are you talking about Submenues? They already exist. Only disadvantage: no drag'n'drop possible to entries in a submenu. And also: can's set an Icon for it. With text labels turned off you'll have to hover to identify it.
